What is PCOS?

PCOS (or polycystic ovary syndrome) is a hormonal imbalance that is caused by the ovaries creating excess male hormones (androgens). Many women experience PCOS, which is often caused by chronic stress and/or overweightedness. Both of these cause insulin resistance, which then stimulates the ovaries to produce male hormones, producing symptoms such as irregular or no periods, excess facial and body hair, sugar cravings, hair loss and difficulty becoming pregnant. Having PCOS does not mean you cannot get pregnant, as PCOS is a treatable cause of infertility. Treatments for PCOS are often non-hormonal and differ depending on each case.

Diagnosing PCOS

After booking an appointment, you will have an initial consultation with a gynaecologist to discuss your symptoms.

To understand whether you have PCOS, and to guide the best course of treatment, you may undertake the following screening tests:

Blood tests

Specific blood tests that look at your hormones will be able to give an indicator as to whether you have PCOS. Blood tests can also help to rule out any other potential issues that might be causing menstrual issues, and can help inform the right course of treatment.

Ultrasound

A transvaginal ultrasound is the most accurate way to check your ovaries and the lining of your uterus. This involves a small ultrasound wand with lubricant being placed inside the vagina, just like a tampon, to produce images of the inside of your pelvic area.

The transvaginal ultrasound is not painful at all and initially just feels cold due to the lubricant gel. A transabdominal ultrasound can be an alternative option to the transvaginal mode, although it is less accurate than the latter.

  • Heavy periods
  • Irregular or missing periods

  • Excessive hair growth (usually occurs on the face, chest, back and buttocks)

We can work with you to help manage and treat your PCOS. There are many different causes of PCOS, so we will be able to shed light on the cause in your specific case once your screening tests are back.

The next step will be to discuss which non-hormonal or hormonal treatments and lifestyle changes will work for you. These could include:

  • Non-hormonal treatments, such as inositol and metformin, to treat irregular cycles, improve ovulation, treat acne and help you reach your healthy weight
  • Hormonal treatments such as the contraceptive pill, or progestin therapy may also be used to help regulate your periods
  • Medications such as clomifene can be used to assist with ovulation
  • Lifestyle changes such as exercising regularly and following a healthy diet will improve symptoms of PCOS and can increase the effectiveness of medications
