What documents do I need to bring for my child’s appointment?

The following summary outlines the essential documents you must bring for identification when visiting our hospitals for paediatric services.

For your child:

  • Red Book (if applicable)
  • Photo ID (a valid passport with the child’s photo).
  • Birth certificate

For you:

  • A valid passport

We’ll need additional documentation if your name on the passport differs from the birth certificate.

For someone bringing a child on your behalf:

  • A letter from a parent or guardian: if someone else is bringing your child, we require a letter from one of the parents listed on the child’s birth certificate

Please note that this is NOT an admitting hospital. You should take any child who needs urgent care to your local NHS GP or straight to A&E.
